The role!
Position: Family Support Coordinator
Location: Wakefield, MA
Pay rate: $22 per hour
Hours: Full-time, 40 hours

Summary & Responsibilities Family Support Coordinators assist families who have sons and daughters eligible for DDS services (Department of Developmental Services). This person will provide home-based services to families to assist in identifying their needs in a way as to translate their needs into services and community supports.
Responsibilities and Duties for Department of Developmental Services (DDS): - Inform the family and assists them in accessing supports and services that exist within the community.
- Assists families in obtaining all state and federal entitlement benefits such as SSI, CommonHealth clinical/medical supports, and behavioral supports
- Have an understanding of the Massachusetts Special Education Laws
- Assists families in working with collaterals
- Collaborate with other agencies
- Supervise support staff working in families homes
For Department of Children and Families (DCF): - Attend all Intake and Review Meetings at DCF
- Develop and Implement Individualized Family Treatment Plans
- Document progress and prepare reports for DCF review meetings
- Complete discharge summaries for all families and all necessary documents on I Family Net
- Identify services and teach skills to aid parents in keeping, or returning their children home
- Have knowledge of child development
- Sensitivity to poverty and cultural issues
Qualifications - Bachelor's Degree in Social Work, Education, Human Services or a related field is required. Master's Degree position is also available.
- Up to two years of family support case management experience preferred
- A passion and dedication to supporting our people is a must
- Valid Driver's license, reliable transportation, and acceptable driving record
- Must pass company background and reference checks
